Medical appointments

All medical appointments should be arranged for outside of school hours, but we appreciate this is not always possible. To report an absence due to a medical appointment , please contact the appropriate Year Group Co-ordinator:

• Years 7 & 8 - Miss Hickmott - Email - hickmottc@chertseyhighschool.co.uk
• Years 9, 10 & 11 - Mrs Clark - Email - clarkj@chertseyhighschool.co.uk 

Please provide the following information:

• Students Full Name
• Year Group
• Tutor Group
• Date of appointment
• Time of appointment
• Please attached evidence of appointment, for example; copy of appointment card, letter, email or text message.
